As a literacy coach, each year I give the CORE Phonics Survey to the below level and approaching level students at my school. I made this results sheet to easily enter the results of all students so that I can easily analyze and share the data with their teachers. If you are not a coach and would like to use this form to track the results of just your own students, it works just as well.As you enter the score, the cell will turn green if the score is on level, yellow if the score is approaching
